The Center for Morton’s Neuroma, LLC is a concierge medical practice and our online store (mortonsneuroma.com/store) is a retail site (the “Service”) owned and operated by The Center for Morton’s Neuroma, LLC. The terms and conditions of use policy, and the privacy policy, identify what users of mortonsneuroma.com (“Users”) can expect from this site, and what we expect from Users. By accessing any areas of this site, Users are deemed to have accepted The Center for Morton’s Neuroma, LLC terms and conditions of use, and this privacy policy identified throughout mortonsneuroma.com.

Children.

The Site is not directed to children and children are not eligible to use our Site. Protecting the privacy of children is very important to us. We do not collect or maintain Personal Information from people we actually know are under 18 years of age, and no part of our Site is designed to attract people under 18 years of age. You are not required to have an Account to place an order. To place an order on the Site, we will require additional information from you, including your mailing address and phone number. If you participate in a sweepstakes, contest or giveaway on our Site, we may ask you for your e-mail address and/or home number (to notify you if you win or not). We may also ask for first and last names, and sometimes post office addresses to verify your identity. In some situations we may need additional information as a part of the entry process, such as a prize selection choice. These sweepstakes and contests are voluntary.   • Information Collected by Our Servers.To make our Site more useful to you, our servers (which may be hosted by a third party service provider) collect information from you, including your browser type, operating system, Internet Protocol (“IP”) address (a number that is automatically assigned to your computer when you use the Internet, which may vary from session to session), domain name, and/or a date/time stamp for your visit.

This information includes IP addresses, browser type, Internet service provider (“ISP”), referring/exit pages, operating system, date/time stamp, and clickstream data. Forms that you fill out or email communications that you send to us via the email links on our site may be shared with a customer service representative, employee, medical expert or agent that is most able to address your inquiry. We make every effort to respond in a timely fashion once communications are received. Updating and Controlling Your Information.

Once we disclose some of your Personal Information to third parties, we cannot access that Personal Information any longer and cannot force the deletion or modification of any such information by the parties to whom we have made those disclosures.
